Important Forms
Below is a list of the most common forms used when taking a scuba class. Prepare for your scuba class and complete the forms appropriate to your program in advance. Please pay special attention to the Medical Statement as you may need physician's approval prior to any water work.

Medical Statement Required for All Scuba Programs For your safety, your physician will need to sign page two of the form if you have a YES answer to ANY questions. This form must be signed before the start of your class. If you have no concerns and answer NO to all the questions then no physician's signature is required. This form does require a parent or guardian signature for all minors.

Standard Waiver Required for All Scuba Programs This form does require a parent or guardian signature for all minors.

Statement of Understanding Required for All Scuba Programs Please read this form carefully. It lists some of the important rules of scuba diving. This form does require a parent or guardian signature for all minors.

Youth Diving Statement Additional Form for Junior Programs This form does require a parent or guardian signature for all minors. The Youth Diving Flip Chart will be shown during the first class and pool session.

PADI Seal Team Statement For the PADI Seal Team and Master Seal Team This form does require a parent or guardian signature for all minors. Please review the Medical Questions carefully. If there are YES answers or any concerns you will need to have your physician sign the full Medical Statement listed above.
